Grant Renewal Procedures
For the trustees to determine your eligibility for grant renewal, you will need to complete the renewal form.
NOTE: Your most recent OFFICIAL transcript is required for renewal. Please email it to firstname.lastname@example.org or mail to W.B. Small Trust, P.O Box 1034, Cedar Falls, IA 50613.
This information should reach us by June 15 so we can make a decision in regard to the renewal of your grant for the next year.
Remember, it is your responsibility to get the required information to us in a timely manner. We DO NOT send reminders.
